In order to view and print these forms, you must have Adobe Acrobat installed on your computer. If you do not have Adobe Acrobat you can download it now for free.
Please Note: The marriage, birth and death record application forms must be submitted to the Cortland City Clerk's Office at 25 Court Street, Cortland NY 13045 with the fee of $10 per copy and a photocopy of a photo I.D. or a notarized statement. If application is mailed in and fee is paid by money order or certified bank check, the copy will be sent immediately. Personal checks will be held until they clear the bank.
